Five rules the CSC follows

  1. Do not abbreviate. Write “University of the Philippines”, not “UP”; “Civil Service Commission”, not “CSC”.
  2. Dates are dd/mm/yyyy. 5 March 1990 is 05/03/1990. Pick dates from the calendar here and we format them for you.
  3. Never leave a box blank. Write N/A if it does not apply. Turn on “Write N/A in empty boxes” and we do it for you.
  4. Names are surname, first name, extension, middle name, each in its own box.
  5. Tell the truth. Any false entry in the PDS can lead to an administrative or criminal case.

PDS FAQ

What changed in the PDS Revised 2026?

The Revised 2026 PDS restores two columns in Work Experience: Monthly Salary, and Salary/Job/Pay Grade and Step Increment. The CSC adopted it through Memorandum Circular No. 05, s. 2026 to correct the Revised 2025 form. Questions 34 to 40 cover relatives in the office, administrative and criminal cases, separation from service, elections, immigrant status, and indigenous, PWD and solo parent status.

Can I still use the old PDS (2017 or Revised 2025)?

Use the Revised 2026 form. The CSC gave a transition period for the Revised 2025 form, after which the Revised 2026 version is the one accepted. Ask your HR office if you are unsure which one they accept.

Do I need to submit the PDS in Excel?

The CSC guide says the PDS should be accomplished using the MS Excel format from the CSC website and filled out on a computer. Use “Download filled Excel” to get the official Revised 2026 file with your entries, then add your e-signature in Excel. You can also print or save a PDF for your own copy.

What do I write if something does not apply to me?

Write N/A. Do not leave boxes blank. Turn on “Write N/A in empty boxes” and every empty box prints as N/A.

What date format does the PDS use?

Day, month, year: dd/mm/yyyy. For example, 5 March 1990 is 05/03/1990. School attendance uses years only, such as 2012 to 2016.

What is the salary grade format 00-0?

The first number is the salary grade and the number after the dash is the step increment. Salary Grade 11, Step 1 is written 11-1. Private jobs have no salary grade, so write N/A.

Who can be my references?

Anyone who knows your work or character and is not related to you by blood or marriage, such as a former supervisor, teacher or co-worker. Give three, with their address and contact number or e-mail.

Is my information safe?

Yes. Everything you type stays in your browser on this device. It is not sent to DailyTimeRecord.com or anyone else. Use “Clear all entries” when you are done on a shared computer.
